Comments:
The inspector conducted an unannounced mandated inspection from 11:00-1:20. There were 30 children in care with staff maintaining ratio at all times. The cook and the family service worker were also present. Children were observed playing outside, preparing for lunch, eating lunch, and getting ready for and then napping. The lunch met USDA requirements. The center is not administering any medications at this time. Two buses were inspected. Four children's records were reviewed. Violations are identified on the violation notice. Please call me at 276-698-9981 with any questions.

Description: Based on record review, the center did not have all the required information in the children's records. Evidence: 1. The inspector reviewed four children's records. Two of four records (file #1 and #2) did not contain two emergency contacts. One of four records (record #1) did not contain proof of ID.

Plan of Correction: Staff will get the information from the parents and document in the file.
