An unannounced renewal study was conducted with the director, who was present with two additional staff and thirty five schoolagers. The children were observed play a ball game in the gym, eating snack ( fruit cups, graham crackers and water) to include hand washing before and after eating. There were no medications on site. Five children and three staff files were reviewed and determined to be in compliance. There have been no documented injuries/accidents this school year. Fire inspection: 9/15/14 Health inspection: 4/14/16. Arrival time: 4:15 p.m. Departure time: 5:25 p.m.
Standard #: 22VAC40-185-260-A Description: Based on a review of documentation and interview with the director it was determined that there was no documentation of an annual fire inspection report from the fire official. Evidence: The last documented fire inspection report is dated 9/15/14. Plan of Correction: We have repeatedly attempted to get a copy of the most recent fire inspection report ( 10/15) from the fire official/school. To date, no report has been provided to us. We will continue to attempt to obtain a copy of the most recent report.
