During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Technical Assistance:
Beginning February 1, 2026, licensed child day programs, licensed family day systems, and voluntarily registered family day homes shall refer to Chapter 8VAC20-821, General Procedures for the Licensure of Child Day Programs and Family Day Systems and Background Checks. This chapter replaces 8VAC20-770, Background Checks for Child Day Programs and Family Day Systems; 8VAC20-820, General Procedures and Information for Licensure; and 8VAC20-830, Fee Requirements for Processing Applications. Comments:
An unannounced, on-site monitoring inspection was initiated on 2/24/2026 and completed on 2/24/2026. The on-site inspection began at 9:50 am and ended at 10:45 am. The inspector reviewed compliance in the areas listed above. There were (8) children present and (3) caregivers; the point total was (22). The inspector reviewed (5) children?s records and (3) caregiver/household member(s) records.
This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-800-780-B
Description: Annually, the provider shall review with the parent the emergency contact information required in each child?s record to ensure the information is correct and shall obtain the parents signed acknowledgment of the review. Records for child #5, did not have a parent?s signature for an annual review of the emergency contact information. Child #5 had been in care for over a year. It has been over four months without any annual review.

Plan of Correction: The parent will review and update the record.

Standard #: 8VAC20-800-960-E
Description: Bottles shall be refrigerated and labeled with the child's full name and the date, if more than one infant is in care.
The bottles for child # 2 were not labeled and dated.

Plan of Correction: All bottles will be labeled and dated each day.

Standard #: 8VAC20-800-100-A
Description: The provider shall obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance. T
There were no physical exam records available for Child #2, who was in care during this inspection.
There was no physical exam for child #4 who was in care during this inspection and who had been in care for over five months.

Plan of Correction: The parents will schedule an appointment for the child's physical and submit the documents to the provider.

Standard #: 8VAC20-800-60-A
Description: An up to date record for each child enrolled shall be maintained by the provider.
There was no record for child #2, who was in care during the inspection.

Plan of Correction: The parent will complete the enrollment documents.

Standard #: 8VAC20-800-60-B
Description: Children's records are required to contain Proof of the child's age and identity and the names and addresses of previously attended child day care and schools, written authorization for emergency.
The record for Child #4 was missing proof of birth and the medical emergency authorization. The child was present during the inspection and had been in care for five months.

Plan of Correction: The parent will complete the enrollment documents.

Standard #: 8VAC20-800-70-A
Description: Before the child's first day of attendance, parents shall be provided required information in writing.
The parents of child # 2 have not received the required information. The child was present during the inspection.

Plan of Correction: The parent will complete the enrollment documents.

Standard #: 8VAC20-800-180-A
Description: A tuberculosis (TB) screening is required every two years from the last screening.
An updated TB screening for Caregiver #3 was not completed and was overdue by four months.

Plan of Correction: Staff will obtain the TB screening.
