Comments:
A monitoring inspection was conducted from 9:45am-11:00am. Upon arrival there were 32 children in care with 6 staff present. A ample of 3 staff and 4 children's records were reviewed. The center did not have any medications. Children were observed during a lesson on shapes and playing with toys.

Violations:
Standard #: 22VAC40-185-70-A
Description: Based on a record review the center failed to ensure all required information was kept for each staff person. Evidence: Staff #2, hired on 08/18/16, did not have a job title listed in the record.

Plan of Correction: This was an oversight. Staff #2's job title was documented in the record during the inspection.

Standard #: 22VAC40-185-270-A
Description: Based on observation the center failed to ensure areas and equipment of the center, inside and outside, were maintained in a safe condition. Evidence: On the playground the free standing basketball hoop was rusted and the base was no sturdy posing a tipping hazard.

Plan of Correction: The director will contact the owner to have the basketball hoop painted and secured.
