Comments:
An unannounced renewal inspection was conducted on September 15, 2016, between the hours of approximately 10:08 a.m. and 2:30 p.m. Twenty-four (24) children were enrolled and twenty (20) children were in the care of three staff members, in two groups, during the on site inspection. One additional staff member was present and assisted, as needed. Three children's records were reviewed and five staff records were reviewed and/or updated. No medications were on site/administered on this date. Posted information (license, licensing inspection documentation, emergency evacuation diagram and plan, emergency telephone numbers, menu and classroom schedule), safety equipment (first aid kit, flashlight and radio) and available documents (emergency evacuation drill and shelter-in-place drill records, insurance, local agency reports and injury reports) were reviewed. The daily schedule accurately reflected the activities provided. Lunch was provided by the children's parents. Program observation was completed. Activities observed on this date included: indoor play with classroom materials/toys and learning activities; outdoor play (including painting paper apples); diapering, restroom and handwashing procedures; lunch; stories; and naptime. Staff/child interactions were observed to be positive and supportive of children's growth and development. The facility was observed to be clean and supplied with a variety of age/stage appropriate learning and play materials/toys. Children's artwork was displayed. A previously cited violation was reviewed for compliance (3/29/2016 inspection date); one repeat violation were cited. The exception to compliance was noted on the Violation Notice. Standard #: 22VAC40-185-160-C
Description: Repeat violation Based on review and/or update of five staff records and discussion with staff #1 on September 15, 2016, the facility failed to ensure that staff obtained and submitted the results of follow-up tuberculosis (TB) screenings at least every two years from the date of the first initial screenings or testing. Evidence: 1. Update of staff #2 and staff #3's records, on this date, determined that TB updates had not been obtained and the results submitted. The date of staff #2's TB report was 4/02/2014 and the date of staff #3's TB report was 5/24/2014. 2. Staff #1 confirmed the dates of the staff members' TB exam/screening reports.

Plan of Correction: The staff members will obtain and submit the results of follow-up TB exams/screenings. In the future, the results of TB follow-up screenings/exam reports will be obtained and submitted at least every two years from the date of the first initial screening or testing.
