An unannounced complaint investigation and monitoring inspection were initiated on 7/28/16 at 2pm. The complaint investigation was in response to concerns regarding the cleanliness of the center, staff turnover and a possible bug infestation. Careful observations were made and interviews were conducted. The preponderance of evidence gathered during the investigation does not support the allegations, so the complaint is determined to be "not valid". Monitoring inspection on 7/28: The physical plant, 4 staff files, 4 children's files, annual fire inspection, staff training, children's activities, medications, emergency procedures, drills and supplies were inspected today. There were 24 children under the supervision of 8 teachers. The children were observed taking naps, completing their workbook assignments (math and writing) and coloring pictures. The interactions between the children and staff were seen to be positive and constructive in nature. There is currently 1 staff certified in MAT (medication administration training). Standard #: 22VAC40-185-70-A Complaint related: No Description: Based on file review on 7/28, 1 out of 4 staff records does not have all required documentation. Evidence 1. Staff #3 does not have a name, address and telephone number of a person to be notified in an emergency. Plan of Correction: 9/19 The director will have staff #3 complete this.
Standard #: 22VAC40-185-270-A Complaint related: No Description: Based on observations on 7/28, there were areas of the center which were not maintained in a safe, operable condition. Evidence 1. In the upstairs bathroom located in the classroom, there was a toilet with a broken/missing toilet seat. There was no method in place to prevent the children from using this toilet. 2. On the wooden gate, which opens into the playground area, there was an exposed nail by the handle. 3. There were 4 wooden slats to the playground fence which were detached or broken. Plan of Correction: 9/19 The director stated all of these violations have been fixed.
Standard #: 22VAC40-185-280-B Complaint related: No Description: Based on observations on 7/28, there was a cleaning material/hazardous substance left out in an area accessible to the children in care. Evidence 1. In the girls' bathroom downstairs, there was a bottle of Febreze left out on top of the toilet in the last stall. Plan of Correction: 9/19 The director spoke to the teachers about this and this was removed.
Standard #: 22VAC40-185-330-B Complaint related: No Description: Based on observations on 7/28, there was insufficient resilient surfacing under and in the fall zones of playground equipment. Evidence 1. There was less than six inches of mulch under (and in the fall zones) of the swings on the playground area. Plan of Correction: 9/19 Addditional mulch (3-4 yards) was added to this area. The existing mulch was also redistributed to areas under playground equipment.
