Technical Assistance:
Staff files and key. Staff who are qualified as leads need to have information recorded and they need to be aware too. Fire Permit has past date but current inspection report.

An unannounced inspection was conducted from 930-11:00. The Inspector arrived and noted the following ratios: 3 years 11 to 2, infants 8 to 2, toddlers 8 to 2, toddler and twos 13 to 2, and PreK is 9 to 1. The Inspector reviewed medication procedures, children's files, attendance lists, and menu. The Inspector also observed programs in the toddler and Pre K group. The threes were also observed playing with play dough and the Inspector noted the new carpet and new materials in the preschool group. The Inspector also observed supervision of children as they went to the bathrooms and reminding the children about hand washing. Please call with questions 703-479-4679

Description: Based on interviews and review of 5 children's files, the center is not maintaining complete records. The following issues were found: 1. Child #2's immunization has not been updated in the last 6 months. 2. Child #3 began attending in May, 2016 and file does not have emergency contacts, physical or immunization record, and proof of birth documentation. 3. Child #4 began attending in June, 2016 and file does not contain addresses for emergency contacts, physician information, proof of birth documentation, and no physical or immunization record.

Plan of Correction: All files are being updated and all have been notified three weeks ago about information needed. All parents have till the 16th to complete.

Standard #: 22VAC40-185-280-B
Description: Based on observation the center has not kept cleaning materials in a locked closet to prevent access. The closet, which has various cleaning materials, is between the toddlers and the threes class and was found unlocked.

Plan of Correction: Corrected during the inspection. Will put a big red sign on the door to remind the staff.

Standard #: 22VAC40-185-350-C
Description: Based on observation, interviews, and record review, the center is not maintaining a 5 to 1 ratio in a group that has a toddler and two years of age children. There are 13 children in the group with 2 staff members.

Plan of Correction: We thought the child was ready as child turns two next week. Parents wanted child up and we all agreed ready but did not document okay. Will have lead and parents sign child is developmentally ready to move up.

Standard #: 22VAC40-185-550-A
Description: The emergency preparedness plan is not up to date as it does not contain a current list of emergency officer and back up officer.

Plan of Correction: We will update the information. And place sign up on parent's board.
