Thank you for your time and assistance with the unannounced complaint inspection which was conducted on September 9, 2016. The on-site inspection was conducted from 8:00 a.m. to 10:25 a.m. At the time of arrival there were nine children in the care of two staff members and upon leaving there were 16 children in the care of four staff members. There were 17 children enrolled in the program. The inspection was in response to a complaint that was received in the Valley Licensing Office on September 7, 2016. The complaint was in regards to the supervision of children in care. The preponderance of the evidence gathered during the investigation did support the allegation and the complaint disposition was, therefore, "valid." The violation issued was noted on the violation notice and the risk rating for the violation was noted on the supplemental page. There were two staff members interviewed and no records were reviewed. Description: Based on observation and verification from the director, the center failed to have enough staff to meet the required staff-to-children ratio for a mixed age group of children. Evidence: 1. At 8:00 a.m., there were nine children together in a mixed age group with two staff members. The youngest child in the group was a ten month-old infant which required a staff-to-children ratio of one staff member to every four children. At 8:15 a.m., the staff divided the children into two smaller groups and one staff member took the five youngest children into the infant room. The center did not obtain the correct staff-to-children ratio until a third staff member arrived at the center at 8:35 a.m.

Plan of Correction: The director stated that she would have a third staff member come in each morning at 6:00 a.m. and if a parent tried to drop off a child unexpectedly and there was not enough staff present, she would have the parent stay with the child until another staff member arrived.
