Comments:
An unannounced monitoring inspection was conducted from 10:00am-12:00pm. Upon arrival there were 47 children in care with 8 staff present. A sample of 4 children and 2 staff records were reviewed in addition to 3 medications. The playground was not inspected due to the flooding in the area earlier that week. Children were observed watching television, playing in centers and during dance.

Violations:
Standard #: 22VAC40-185-430-I
Description: Based on observation the center failed to ensure personal articles were individually assigned. Evidence: There was a bottle of lotion in a supply cabinet in the front preschool room.

Plan of Correction: The lotion was labeled during the inspection.

Standard #: 22VAC40-185-510-A
Description: Based on observation the center failed to ensure all requirements were met for prescription medication. Evidence: Child #5 had an emergency medication that did not have parental authorization to administer.

Plan of Correction: The director will have the parent sign the authorization form today at pick-up.
