An unannounced monitoring inspection was conducted today from 4:50pm-6:10pm. There were 43 children directly supervised by 4 staff. The physical plant, 4 children?s records, 2 medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play and doing crafts. There was a sufficient supply of materials for the children. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-280-B Description: Based on direct observation, a hazardous substance was not kept in a locked place, preventing access to children. Evidence: 1. A cleaning solution was observed on a table, in the common area, and was accessible to children. Plan of Correction: The substance was locked away during the inspection.
Standard #: 22VAC40-185-550-M Description: Based on record review, 2 of 2 injury reports did not have all required information. Evidence: 1. Child #1 and #2 did not have a documented action plan, to prevent injury recurrence. Plan of Correction: I will review with staff what is needed on the form.
