An announced initial inspection was completed on 9/27/16. The physical plant areas were observed inside and outside, the set-up of the classroom was observed, emergency supplies (first aid kit, flashlight, radio, emergency maps/procedures posted) were observed. Measurements of the facility were made on 7/22/16. Board officer background checks were reviewed on 9/6/16. The teacher's record had been previously reviewed. The inspector arrived at the center at 1:30 pm and departed at 3:45 pm. There was discussion with the director about the following: temperature of the hot water, a wooden board on the back door ramp to watch for possible maintenance, the emergency preparedness plan for this site.

Description: Based on observation, the center failed to ensure that outside lighting was provided at entrances and exits used by children before sunrise or after sundown. Evidence: 1. The outside light at the entrance and exit doors both in front and back were not working.

Plan of Correction: The family service manager will report this to the church maintenance person to have it fixed. In the future, the staff will report any maintenance needs to the church staff to be fixed.
