Description: Based on review of two staff files, the facility failed to have documentation of qualifications of the designated director. Evidence: there was no documentation of the educational component in the file for person designated as director.

Plan of Correction: Staff B has given a copy of degree and transcripts to main office, but there is no information in the site file. Center director will request information from main office.

Standard #: 22VAC40-185-260-A
Description: Based on review of school inspections, the facility failed to have documentation of the most recent fire inspection. Evidence: the last documented fire inspection is dated 3/12/2015.

Plan of Correction: Center staff will check with school office to see if there is an updated fire inspection.

Standard #: 22VAC40-191-60-C-2
Description: Repeat violation: Based on review of two staff records, the facility failed to have documentation of a search of the central registry in each staff's file. Evidence: Staff A (date of hire 8/13/2013) did not have documentation of search of the central registry check in the file on site.

Plan of Correction: Staff A can not return to work until documentation of a completed search of the central registry (CPS check) is provided to licensing inspector.
