Comments:
Today's visit was a routine monitoring inspection. The inspection began at 10:00am and concluded at 4:30pm. There were sixty-seven children present with seventeen staff members and arrival of the school age children was observed to include thirty-two children. During the inspection all nine classrooms were viewed, diapering was observed, lunch time was observed, hand washing was observed, four out of one hundred thirty children's records were reviewed, medication was viewed, some policies and procedures were reviewed, and emergency supplies were viewed. The playground was not viewed due to rain. Thank you for your time, please call my office if you have any questions.

Violations:
Standard #: 22VAC40-185-130-B
Description: Based on the inspector's review of four out of one hundred thirty staff records, the center failed to ensure a record had an update immunization record. Evidence: The record for child #2 is under the age of one and has an immunization record dated 12/15/15. The immunization record is required to be updated every six months for children under the age of two.

Plan of Correction: The PD has previously notified the parent and requested a copy of the child's current immunization record. A second request will be made.

Standard #: 22VAC40-185-520-A
Description: Based on the inspector's review of four diaper creams in the infant/waddler classroom, the center failed to ensure a diaper cream was kept beyond the date of expiration. Evidence: Four diaper creams were reviewed in the infant/wadler classroom and one of the diaper creams has an expiration date of 7/2016.

Plan of Correction: The PL for the classroom will send the diaper cream home with the parent at the end of the program day. The classroom will monitor dates of expiration on a weekly basis.

Standard #: 22VAC40-185-520-C
Description: Based on the the review of four diaper creams in the infant /waddler classroom, the center failed to ensure a diaper ointment was labeled with the child's name. Evidence: The inspector reviewed four diaper creams in the infant/waddler room and one diaper cream was not labeled with the child's name of whom it belonged to.

Plan of Correction: The diaper cream will be labeled by the end of the program day. Diaper creams will be checked at least weekly to ensure they are properly labaled.

Standard #: 22VAC40-191-60-C-1
Description: Based on the inspector's review of eleven out of thirty-two staff records, the center failed to ensure results of criminal background checks were received within thirty days of employment. Evidence #1: The record for staff member #3 with a documented date of hire as 8/19/16 and there are no results for the CRC. Follow-up was not documented with the agency responsible for processing the background check. The staff member has been employed for more than thirty days. Evidence #2: The record for staff member #4 with a documented date of hire as 8/22/16 and there are no results for the CRC. Follow-up was not documented with the agency responsible for processing the background check. The staff member has been employed for more than thirty days.

Plan of Correction: The PD Contacted the responsible agency during the day of the inspection. In the future the PD will contact the responsible agency at the thirtieth day if not returned. Results will be documented in the staff members records.

Standard #: 22VAC40-191-60-C-2
Description: Based on the inspector's review of eleven out of thirty-two staff records, the center failed to ensure results of the search of central registry checks were received within thirty days of employment. Evidence #1: The record for staff member #3 with a documented date of hire as 8/19/16 and there are no results for the CPS background check. Follow-up was not documented with the agency responsible for processing the background check. The staff member has been employed for more than thirty days. Evidence #2: The record for staff member #4 with a documented date of hire as 8/22/16 and there are no results for the CPS background check. Follow-up was not documented with the agency responsible for processing the background check. The staff member has been employed for more than thirty days.

Plan of Correction: The PD Contacted the responsible agency during the day of the inspection. In the future the PD will contact the responsible agency at the thirtieth day if not returned. Results will be documented in the staff members records.
