Technical Assistance:
**Spoke with staff about expired medications in the medication box, and letting them know that the expired meds needed to be returned to the parents, an new medication needs to be obtained as soon as possible.

An unannounced monitoring inspection was conducted today at 2:05pm. There were 32 (infants-5yrs.) directly supervised by 4 staff. The physical plant, 4 staff records, 6 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed napping and awaking up from nap, cleaning of nap time equipment. There was an abundant supply of books, toys, and materials for the children. The center was clean and organized. Areas of non-compliance are identified in this report. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 10 calendar days from today. Description: Based on review, the facility failed to ensure that children's records of a physical examination were received and signed by a physician, their designee or an official of a local health department. Evidence: Reviewed 6 children's records and found that CH4 did not have documentation of a signed physical examination.

Description: Based on observation, the facility failed to ensure that children's records are kept confidential. Evidence: Observed that the attendance sheet was kept on the just inside the initial facility doors, the front area that is accessible to the public and does not need a security code to enter. The attendance sheet had first and last names of children, and their attendance for the current day.

Description: Based on review, the facility failed to ensure that staff records contained all required information. Evidence: Reviewed 4 staff records and found that... 1) Staff 1 and 2 did not have documentation of 2 reference checks prior to employment. 2) Staff 3 only had documentation of 1 reference check prior to employment. 3) Staff 2 did not have documentation of orientation training.

Description: Based on observation, the facility failed to ensure that hazardous substances were kept in a locked place using a safe locking method that prevents access by children. Evidence: Observed 3 bottles of shaving cream accessible to children on a low shelf in the 3 year old room, and a bottle of hand sanitizer accessible to children by chair, in the room next door.

Description: Based on observation, the facility failed to ensure that children's cots were placed at least 12 inches apart. Evidence: Observed 6 cots in the 3 year old room that were placed within 12 inches of each other, while children were napping.
