Thank you for your time and assistance regarding the unannounced investigation that was conducted on July 11, 2016. The on-site visit was conducted from 2:00 p.m. to 4:15 p.m., and it was held in conjunction with the Albemarle County Department of Social Services. The investigation involved a situation that was self-reported by a staff member from the center to the Valley Licensing Office on July 6, 2016. Interviews were conducted with staff and records were reviewed. No violations were found for the incident that was self-reported. One violation of standard 520-C was found. Standard #: 22VAC40-185-520-C Description: Authorization for administering diaper ointment did not contain the parent's signature for Child A. Evidence: Based on record review on 7/20/2016, Child A's diaper administration permission did not have the parent's signature. Plan of Correction: We will review all the children's files who have ointment to ensure all the ointment forms are present and properly signed. We will include a procedure where ointment will not be kept in the center without the form being signed even if the parent will be applying it. If the parent chooses not to sign the form, we will not accept the ointment nor keep incomplete forms on file
