Comments:
An unannounced monitoring visit and facility tour was conducted from 3:38 pm-4:45 pm. At the time of inspectors' arrival there were 27 preschool aged children in care with 9 staff members and 1 administrators. Children were observed participating in instructional time in the classroom, transitioning between activities and toileting and hand washing. A sample of 4 children's records and 3 staff records were reviewed. Medications and inury reports were also reviewed.

Violations:
Standard #: 22VAC40-185-140-A
Description: Based on record review it was determined that the center did not ensure that each child had a physical examination under the direction of a physician within the required time frame. Evidence: The record for child #2(enrolled 9/12/16, DOB 1/5/15) did not contain a physical completed within six months prior to attendance for children aged 19 months through 24 months. The physical documentation in the record was dated 2/25/15.

Plan of Correction: The director will request an updated physical from the parent and file in the child's record once received.

Standard #: 22VAC40-185-160-A
Description: Based on record review it was determined that the center did not ensure that each staff member submitted documentation of a negative tuberculosis screening. Documentation of the screening shall be submitted no later than 21 days after employment or volunteering and shall have been completed within 12 months prior to or 21 days after employing or volunteering. Evidence: The records for staff #2(hired 9/6/16) and staff #3(hired 9/6/16) did not contain documentation of negative tuberculosis screening completed within 12 months prior to employing. The documentation in staff #2's record was dated 7/2/15. The documentation in staff #3's record was dated 5/8/15.

Plan of Correction: The director will request an updated tuberculosis screenings from staff #2 and staff #3 and file in their record once received.

Standard #: 22VAC40-185-290-3
Description: Based on observation it was determined that the center did not ensure that all electrical outlets have protective outlet covers in areas with preschool children. Evidence: In the toddler classroom there was an outlet at floor level with out a protective cover. A 20 month old was in care in this classroom during the inspection.

Plan of Correction: Covered during inspection. Director is having the outlet replaced with a safety outlet.
