An unannounced renewal visit was conducted today. Four staff were present with sixty-two children and within ratios for school-age children. Children were observed transitioning into the after school program, hand washing, accountability check and snack. Snack served today was animal crackers and oranges. A evacuation drill was conducted with the children following snack. Emergency medication was on-site with two staff having Medication Administration Training (MAT) certification as well as first aid and CPR training. The dedicated SACC space is still under development for programming. One injury form was complete and radio and flashlight were working. Areas of non-compliance are identified in this report. Standard #: 22VAC40-185-60-A Description: Based on record review, a child's file is incomplete. Evidence: Child #1 is missing both physical and immunization record. The child's record was transferred to the summer SACC and not returned to the school year site. Plan of Correction: Obtain physical and immunization record from the school.
Standard #: 22VAC40-185-510-A Description: Based on medication review, one of three medications was missing parent authorization. Evidence: Child #2's medication authorization was not signed by the parent giving the center permission to the administer in case of an emergency. Plan of Correction: Have parent sign the form.
