Comments:
An unannounced monitoring inspection was done today September 22, 2016 at the site beginning at 3:15 PM and ending at 5:30 PM. The inspector had previously read staff files on August 30, 2016 at the main office. There were thirty-three children present today during the inspection. The children were observed during outside play, snack and homework time. A sample of children's files were reviewed today. There were no medications at the site today. The outside play area was reviewed and the site director indicated the preschool children do not play on the equipment, only on the blacktop area. Remember to a new fire inspection prior to 10/1/2015 and staff # 2 will need a new TB test prior to 10/9/16. The site coordinator was going to discuss the need for an additional staff person on days when the attendance is over thirty and preschool children are in attendance. The violations and corrective action were discussed today with the site coordinator. If you have any additional questions, please contact your inspector at 540-309-2835.

Standard #: 22VAC40-185-70-A
Description: Based on review of staff files the center did not obtain all of the required information for staff files. Evidence: 1. During the inspection the inspector reviewed five staff files and staff # 1 did not have verification of educational requirements for the position hired.

Plan of Correction: The site director indicated this would be discussed with her supervisor in order for the proper verification be provided by the staff member.

Standard #: 22VAC40-185-350-C
Description: Based on observation the center did not meet the ratio requirement for mixed age groups of children. Evidence: 1. When the inspector arrived at the center at 3:15 PM there were thirty-three children in care with two staff. A third staff member arrived at 3:30PM. No other staff were present during the inspection. Based on discussion with staff # 2 & 3, the youngest child was age three and the oldest was age eleven. A record review verified that the youngest age child in care was actually age four. An fourth staff member was needed to met the required ratio during the inspection as long as there was over thirty children in care.

Plan of Correction: The site director indicated this would be discussed with her supervisor in order for staff changes be done in order to be in compliance.

Standard #: 22VAC40-191-40-D-1-C
Description: Based on review of staff files the center did not obtain the required repeat background checks on all staff. Evidence: 1. During the inspection the inspector reviewed four staff files and staff # 1 was in need of a repeat sworn disclosure form as the last one in the file was dated 2/7/13.

Plan of Correction: The director indicated she would contact her supervisor to have this completed.
