An unannounced monitoring inspection was conducted from 10:28 a.m. - 2:55 p.m. There were forty-three children in care with eight teachers working directly with them. There were two additional staff available to assist as needed. Three children's records were reviewed. The record for one teacher was reviewed. Previous violations were reviewed for compliance. The infants were observed during diaper changing, hand washing, having lunch and being played with and talked to by staff. The younger children were observed sitting at the table practicing good manners while having lunch. The older children were observed playing and talking and laughing with one another and their teachers, during clean-up and preparing to go outside to play. Standard #: 22VAC40-185-550-M Description: Based on a review of injury records the facility failed to ensure documentation of any future action to prevent recurrence of injury. Evidence: 1. On 10/07/2016 the licensing inspector reviewed injury reports and noted there was no documentation of any future action to prevent recurrence of injury. 2. On 10/07/2016 staff #1 reviewed the injury form and confirmed there was no documentation of any future action to prevent recurrence of injury. Plan of Correction: I corrected the form immediately and shredded the old forms that I believe were put there for use.
