Comments:
A complaint was received by The Central Regional Licensing office on September 14, 2016 concerning Nutrition and food services. The licensing Inspector conducted an unannounced inspection in response to the complaint on 10/06/2016. The facilities owner and staff were interviewed, health inspection reports and menus reviewed along with a lunch time observation in two classrooms. Based on the information gathered during the investigation, the preponderance of evidence does not support the allegation. Therefore the complaint is determined to be not valid. Description: Based on observation, the facility did not ensure a menu listing foods to be served for meals and snacks during the one-week period was dated. Evidence: A menu was posted on the parent board but was not dated. The menu listed a "sandwich" but did not list what kind of sandwich. The menu for the week must be dated and list foods to be served.

Plan of Correction: Per the Director: I will date each weekly menu and list the sandwich fillers on the menu.
