An unannounced monitoring inspection was conducted with the director beginning at 3:15 PM and ending at 5:00 PM. There were thirty-one children in care ages ranging from five to twelve years old. The children were observed eating snack which consisted of a fresh apple and milk. The children were also observed during homework time and outside recreation time. Eight children's records and two staff records were reviewed. Seven medications were reviewed and determined to be in compliance. The first aid kit, flashlight, and weather band radio were reviewed and determined to be in compliance. The most recent emergency escape drill was conducted on September 19, 2016. The most recent fire inspection was conducted January 12, 2016. The most recent health inspection was conducted August 31, 2016. Standard #: 22VAC40-185-160-A Description: Based on review of two staff records, the center did not have documentation of all staff member's negative tuberculosis screening completed within 12 months prior to or 21 days after employment. Evidence: The record for staff A with a hire date of August 13, 2016 did not have documentation of a tuberculosis screening. Plan of Correction: The staff stated that she completed the tuberculosis screening and turned it into the human resource department. The director will contact human resources for a copy of the screening.
Standard #: 22VAC40-191-60-C-2 Description: Based on a review of two staff records it was determined that not all staff records had central registry findings within 30 days of employment. Evidence: The record for staff A with a hire date of August 13, 2016 did not have a central registry finding. Plan of Correction: The director will contact human resources to follow up to see if the central registry check was completed. The staff will not be able to return until the central registry check is obtained.
