10/17/2016 Arrived 10am Departed 10:55am An unannounced monitoring inspection was conducted today. Eleven children were present with two staff. The children were working individually with science projects, various manipulatives, drawing and having group time. Two staff files, and two children's files, were reviewed. Standard #: 22VAC40-185-270-A Description: Based on a our of the playground the facility failed to maintain all areas in a safe condition. Evidence: 1. The wood chips used for resilient surface material are not uniform in size. 2. Some pieces are 3-5 inches long and 1-2 inches wide. There are sharp, pointed ends and splinters. Plan of Correction: The director reported that the larger, sharp pieces of wood mulch will be removed from the playground.
