Plan of Correction: I will have her fill out another.

Description: Based on record review, 1 of 2 injury reports were incomplete. Evidence: 1. Child #1's injury report for 10/7/16 did not have a staff and parent signatures or two staff signatures.

Plan of Correction: Will make sure it is filled out on site.

Description: Based on direct observation, 3 of 3 children, did not have their food container brought from home dated and labeled. Evidence: 1. Children #2-4 did not have a name and date on their food container brought from home.
