Comments:
An unannounced monitoring inspection was conducted on October 18, 2016 from approximately 3:35 PM to 5:45 PM. Upon arrival to facility, five staff were present with forty-seven children. Additional staff were on site for administrative duties. Children were observed playing organized games and participating in character trait discussion groups. Licensing Inspector reviewed standards related to supervision, activities, equipment, arrival procedures, children and staff reports, injury reports, emergency evacuation and shelter-in-place drill documentation, liability insurance coverage, fire and health inspection reports, emergency supplies and required postings. Violations were reviewed and verified by Program Director.

Standard #: 22VAC40-185-160-A
Description: Based on review, facility did not ensure 1 of 4 staff records has a TB test/screening within 21 days of beginning employment. Evidence: On October 18, 2016, the record for staff #1 (start date September 16, 2016) did not have a TB test/screening.

Plan of Correction: Program Director will tell staff that she will be taken off the schedule if the report is not provided.

Standard #: 22VAC40-185-260-B
Description: Based on review, facility did not ensure there is an annual health inspection. Evidence: On October 18, 2016, the most recent health inspection report was completed July 8, 2015.

Plan of Correction: Program Director will contact the health department tomorrow to request an inspection.
