Comments:
An unannounced monitoring inspection was completed on October 19, 2016 from 10:08 to 12:26. The purpose of this visit was to measure the provider's compliance with licensing standards. There were 19 children in care with 3 staff persons. The children ranged in age from 2 years to 5 years old. The children were observed playing with blocks, toys, examining rocks with a magnifying glass and playing in the outdoor playground. Lunch consisted of pizza, salad, fruit cup, corn and milk. A sample of staff and children's records were reviewed. The last evacuation drill was conducted 9/29/16. The last shelter in place drill was conducted 7/19/16. The annual health inspection was conducted 7/25/16. The annual fire safety inspection was completed 3/9/16.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observations of the outdoor play area, the facility did not ensure that the playground area is maintained in safe condition. EVIDENCE: Two rusty nails protruding from the wooden fence, accessible to children, which could entangle clothing or snag skin.

Plan of Correction: Will remove nails from fencing area on 10-19-16.

Standard #: 22VAC40-185-290-3
Description: Based on observation of the toddler room, the facility did not ensure that electrical outlets have covers. EVIDENCE: An outlet, accessible to the children in the toddler room, that did not have a protective cover.

Plan of Correction: Protective cover has been replaced on 10/19/16.

Standard #: 22VAC40-185-380-A
Description: Based on observations of the toddler and preschool classrooms, the facility did not ensure that daily schedules were posted. EVIDENCE: There was no daily schedule posted in either the toddler or preschool classrooms.

Plan of Correction: Daily activity schedule was posted on 10/19/16.

Standard #: 22VAC40-185-510-G
Description: Based on a review of records, the facility did not ensure that medication authorizations were on file for all children with medication on site. EVIDENCE: Child #1 and Child #2 did not have medication authorizations on file at the facility for the administration of their medications that were kept on site.

Plan of Correction: Parents will be requested to have documentation by Tuesday, October 25, 2016.
