Children were napping upon arrival and were eating snack as the inspection ended. Children were observed during hand washing procedures and in conversation with each other as well as the teachers. Standard #: 22VAC40-185-160-A Description: Based on review of records, it was determined that 1 out of 3 staff records did not have documentation of a negative tuberculosis screening available for review. Evidence: Staff #1 was hired on 12/2015 and there was no documentation of the screening in the file. Plan of Correction: TB screening was complete and on file at time of inspection, but was missed. Copy is attached.
Standard #: 22VAC40-185-270-A Description: Based on observations of the playground, the facility did not maintain clean, safe and operable areas and equipment. Evidence: 1. The wood surrounding the plumbing, on the wooden water station located on the playground, has become unattached and is exposing the sharp end of two screws. 2. The outside red door, leading from the playground into the toddler/infant side of the building, contains a large area of peeling paint. Plan of Correction: We had previous work orders placed for these items and have resubmitted them as urgent. We have also notified heads of facilities for our region and provided copies of the inspection.
