An unannounced monitoring inspection was conducted on October 19, 2016 from approximately 9:10 AM to 12:00 PM. Eight staff were present with forty children during the tour of the facility. Additional staff were on site for administrative and kitchen duties. Children were observed during circle time and outside play. Licensing Inspector reviewed standards related to supervision, activities, equipment, diapering procedures, special feedings, nutrition, children and staff records, emergency evacuation and shelter-in-place drill documentation, fire and health inspection reports, injury reports, emergency supplies and required postings. Violations were reviewed with and verified by the program director.
Standard #: 22VAC40-185-140-A Description: Based on review, facility did not ensure 1 of 4 children's records has a physical within a month of beginning attendance. Evidence: On October 19, 2016, the record for child #2 (start date January 11, 2016) did not have a physical. Plan of Correction: Will have the physical. If unable to do so, will inactivate the child until physical is done.
Standard #: 22VAC40-185-160-C Description: Based on review, facility did not ensure 2 of 3 staff have updated TB test/screening every 2 years. Evidence: On October 19, 2016, the following was found: 1. The record for staff #1 has a most recent completed TB test dated August 19, 2013. Documentation stated a test was completed on December 1, 2015 however no results were noted. 2. The record for staff #2 has a most recent completed TB test dated October 9, 2014. Plan of Correction: Employee will have completed TB test.
Standard #: 22VAC40-185-60-A Description: Based on review, facility did not ensure 2 of 4 children's records have documentation of required information. Evidence: On October 19, 2016, the records for child #1 (start date July 26, 2016) and child #4 (start date April 9, 2015) did not have documentation of father's home information, father's employment information and emergency contact addresses. Plan of Correction: The information will be provided by parent.
Standard #: 22VAC40-185-550-M Description: Based on review, facility did not ensure 3 of 3 injury reports have documentation of required information. Evidence: On October 19, 2016, 3 injury reports completed in September 2016 did not document the time parent was notified of the injury. Plan of Correction: Program Director will retrain staff on completion of incident reports.
