Comments:
An unannounced inspection was initiated in the facility on August 10, 2016 regarding staffing and supervision and special care provisions and emergencies. The inspection included observations, a review of center documentation and interviews with staff members and parties involved. Based on information obtained during the inspection the preponderance of evidence does support the allegation related to special care provisions and emergencies and staffing and supervision therefore the complaint is determined to be VALID. Areas of non compliance are noted on the violation notice. Please complete the columns for description of action to be taken and date to be corrected for each violation cited on the violation notice, and then return a signed copy to the licensing office within 10 calendar days of receipt. Description: Based on staff interviews and attendance documentation, the licensee did not ensure that for infants, there was one staff member for every four children. Evidence: 1. During the inspection conducted on August 10, 2016, interviews conducted with staff members in charge confirmed there are ratio problems in the morning. 2. Reviewing documentation from August 2, 2016 there was 2 staff left alone with 9 children from approximately 8:25 am - 8:30am.

Description: Based on staff interviews and attendance documentation, the licensee did not ensure that for two-year old children, there was one staff member for every eight children. Evidence: 1.During the inspection conducted on August 10, 2016, interviews conducted with staff member in charge confirmed there are ratio problems in the morning. 2.Reviewing documentation from August 2, 2016 one staff member was left alone with 14 children from ages 2 years to 12 years from approximately 6:27 am - 7:00 am.

Description: Based on staff interviews and attendance documentation, the licensee did not ensure that for children from three years to the age of eligibility to attend public school, five years by September 30: one staff member for every 10 children. Evidence: 1. During the inspection conducted on August 10, 2016, interview conducted with staff members in charge confirmed there are ratio problems in the morning. 2. Reviewing documentation from August 2, 2016 one staff member was left alone with 11 to 13 children from approximately 8:00 am - 8:20am.

Description: Based on review and staff interviews, the licensee did not ensure appropriate officials notify the parent immediately if a child sustains a serious injury. Evidence: 1. During the inspection conducted on August 10, 2016, the staff did not notify the parent of Child #1 immediately when the child sustained a serious injury while in the care of staff members at the center. 2. According to documentation, the child sustained the injury at approximately 10:00am and through staff interviews it was determined that the parent was not notified until the child was picked up at approximately 5:30pm. Child did receive medical care on August 2, 2016.

Description: Based on review, the licensee did not ensure that a written record of a child's injury was documented with all required information. Evidence: During the inspection conducted on August 10, 2016, an injury report dated August 2, 2016 did not include documentation of the time and how that the parents were notified of the injury and treatment to the injury.
