Technical Assistance:
Technical assistance was given regarding standard 22VAC40-185-(2)-140-B related to physical examinations for children conducted by a physician or designee prior to the child's first date of attendance and the required time frames for conducting the physicals. Technical assistance was also given regarding the clear documentation of complete addresses as they relates to emergency contact information for staff.

Comments:
An unannounced inspection was conducted October 24, 2016. At the time of entrance there were 28 preschool children in the "Kids" program room and 4 staff members present. The preschool children were observed during their rest period and preparing for snack. There were 12 (teens) children with 2 staff members observed playing in the "Teen" program room and they were observed working on homework and computers. The sample size of 5 children's records were reviewed. There were six medications reviewed. The inspection began at approximately 2:15 p.m. and ended at approximately 4:15 p.m.. Areas of non-compliance are identified on the violation notice. The results of the inspection and violations cited were reviewed and verified with the Recreation Center Supervisors for the "Kids" program ans the"Teens" program during the exit interview. Please complete the columns for ?description of action to be taken? and ?date to be corrected? for each violation cited on the violation notice, and then return a copy to the licensing office within 10 calendar days of receipt. Violations:
Standard #: 22VAC40-185-150-B
Description: Based on a record review and staff interviews, the licensee did not ensure each immunization and physical report included the date of the physical examination and dates immunizations were received and was signed by a physician, his designee, or an official of a local health department. Evidence: During the inspection on October 24, 2016, the record for child #4 did not include an immunization record that was signed by a physician, his designee, or an official of a local health department.

Plan of Correction: The center supervisor will ensure each physical is signed by a physician designee or health depart.

Standard #: 22VAC40-185-280-B
Description: Based on observation and staff interviews, the licensee did not ensure hazardous substances such as cleaning materials, insecticides, and pesticides are kept in a locked place using a safe locking method that prevents access by children. Evidence: During the inspection on October 24, 2016, on a shelf in the unlocked office of the preschool room there were 4 hazardous chemicals observed which were labeled "keep out of reach of children" and at least one of the other statements "caution', "flammable" and "warning". The chemicals observed were one container of disinfectant wipes, two containers of antibacterial soap and one container of hand sanitizer.

Plan of Correction: The center supervisor will ensure the office door is locked when not in use.

Standard #: 22VAC40-185-550-L
Description: Based on documentation review and staff interviews, the licensee did not ensure all the required information was maintained for a record of children's serious and minor injuries in which entries are made the day of occurrence. Evidence: During the inspection on October 24, 2016, five injury reports were reviewed and one did not include the two required signatures, the time the parent was notified and how the parent was notified of the injury.

Plan of Correction: The center supervisor will ensure they are using the updated accident/incident log with all information needed.
