1. Reviewed medication authorization regulations.
Conducted an unannounced monitoring inspection 12:05-1:45pm. Observed 44 children + 11 direct-care staff. Ratios and supervision were in compliance. Children were engaged in a variety of activities during the inspection to include: eating lunch, reading a story, washing hands, and taking naps. Infants were pushed in strollers for an outdoor walk, being fed and diapered, and were taking naps. Classrooms were found to be very clean, safeguarded appropriately, and well supplied with developmentally appropriate toys and equipment. The outdoor playground was not inspected due to inclement weather. Staff were actively engaged with the children, assisting them with tasks as needed, and promptly tending to their needs. Standard #: 22VAC40-185-160-A Description: Based on records reviewed, it was determined that 1 of 3 staff records did not include a negative TB screening. Evidence: Staff #6 - Date of hire was February 2014, and there was no documentation of a TB screening on-file. Plan of Correction: received negative TB on 10-6-16
Standard #: 22VAC40-185-510-G Description: Based on observations made, it was determined that a prescription medication was not maintained in the original labeled container. Evidence: A medication belonging to Child #1 was not in the original medication box or have a prescription label affixed. Plan of Correction: send medication home
Standard #: 22VAC40-185-550-M Description: Based on records reviewed, it was determined that 4 of 4 written injury reports did not include required information.
