Description: Based on direct observation, a hazardous substance was not kept in an unlocked place. Evidence: 1. A cleaning material was stored under the classroom's pet cage. The bottle was located on the floor and accessible to the children.

Plan of Correction: The substance was locked away, during the inspections.

Description: Based on direct observation , 2 of 2 classrooms, with 2 year old children, did not meet the 1:8 ratio. Evidence: 1. Classroom 1 had a 2:22 ratio (including child #1 D.O.B. 01/7/14) 2. Classroom 2 had a 2:23 ratio (including child #2 D.O.B. 11/6/13)
