An unannounced monitoring inspection was conducted with the director beginning at 3:45 PM and ending at 5:15 PM. There were twenty children in care ages ranging from eleven to thirteen years old. The children were observed completing homework and constructing 3D worlds on the computers. Children were also observed eating snack which consisted of turkey sandwich and fruit juice. Four children were observed arriving from after school extra-curricular activities. Eight children's records and two staff records were reviewed. Eight medications authorizations and logs were reviewed. The first aid kit, flashlight, and weather-band radio were reviewed and determined to be in compliance. The most recent emergency escape drill was conducted on October 12, 2016. The most recent fire inspection was conducted on January 16, 2016. The most recent health inspection was conducted on November 10, 2016. Standard #: 22VAC40-185-60-A Description: Based on a review of six children's records it was determined that not all of the records were complete. Evidence Child B's record did not contain two emergency contact Plan of Correction: The center will request the missing information from the parents.
Standard #: 22VAC40-185-510-A Description: Based on a review of eight medications it was determined that not all the medications had written authorization. Evidence: Child A?s long term emergency medication did not have written authorization from a physician. Plan of Correction: The center will request that the parent complete an updated 10 day authorization for the medication and obtained the physician's authorization for long term medication.
