Comments:
An unannounced monitoring inspection was completed today 10/28/2016. There were 33 children in care on the day of the inspection with eight staff. There were five children's and four staff's records reviewed during the inspection. A physical plant walk through was completed, and observations of the children. Discussion: car seat requirements for all children, and lead teachers and grouping of children.

Standard #: 22VAC40-185-330-B
Description: Based on observation and measurements, the center failed to ensure that all climbing structures had the required amount of resilient surfacing. Evidence: 1. The inspector found that a climber did not have the required six feet around the entire piece. The sides of the equipment measured three to four feet and the required is six feet of resilient surfacing.

Plan of Correction: The director will remove the piece of equipment.

Standard #: 22VAC40-185-340-D
Description: Based on review of four staff's records and observation of the infant and toddler area, the center failed to ensure that each grouping of children had a program leader that was regularly present. Evidence: 1. The inspector observed two groupings of children in the infant and toddler area. The four infants were with the lead teacher. Six toddlers were with two aides in a grouping of children with out the lead teacher regularly present. The classroom is divided by a four foot wall and separates the toddlers from the infants. The lead teacher supervised the aides, but were not regularly with the aides and toddlers.

Plan of Correction: The director will work in the classroom until a program lead is qualified.
