Technical Assistance:
Today we discussed triangular bandages, diaper changing surfaces, sun screen adverse reactions, injury reports, and emergency contact information.

Comments:
An unannounced monitoring inspection of the center's facilities and records was conducted from 12:15pm through 2:20pm. There were 51 children present with 11 staff. Children were observed in teacher and self-directed activities including nap, transitions, and restroom routines. The inspector reviewed documentation for emergency evacuation drills, medication, menus, and required postings. Violations regarding labeling infant bottles and sippy cups were observed and are cited on the violation notice. .

Violations:
Standard #: 22VAC40-185-560-G
Description: Based on observation and interview the center failed to ensure that when food is brought from home the containers are labeled with the child's name and date.
Evidence:
1 - In the older infant room there are three sippy cups in the refrigerator. However, the sippy cups are not labeled with the owner's name nor are the sippy cups dated.
2 - Staff 2 confirmed that the sippy cups are not labeled with the owner's name and date.

Plan of Correction: Center management will remind and retrain staff regarding the need to ensure that the child's name and date are clearly printed/written on all food and drink items brought from home into the center.

Standard #: 22VAC40-185-570-E
Description: Based on observation and interview the center failed to ensure that infant bottles are labeled with the child's name and dated.
Evidence:
1 - In the young infant room there is one bottle in the refrigerator that does not have the date on the bottle.
2 - Staff 3 confirmed that the bottles did not have the date on the bottle.
3 - In the middle infant room there are two infant bottles that do not have the child's name or date on the bottles. Additionally, there are two infant more infant bottles that do not have a child's name on the infant bottles.
4 - Staff 4 confirmed that the bottles do not have the child's names and dates on them.

Plan of Correction: Center management will remind and retrain staff regarding the need to ensure that the child's name and date are clearly printed/written on each infant bottle
