Technical Assistance:
All staff should know that Hylton Boys and Girls Club in Dale City is your relocation site in the event you have to evacuate. Please use caution tape to mark the seat on Bus # 4 off limits. All staff needs two references.

Comments:
An unannounced renewal study was conducted today with the assistant director, who was present with two additional staff and two children at the beginning of the inspection. By the end of the inspection 9 children were on site. The children initially gathered in the game room then went into the gym to participate in ball games. Three children had emergency medications on site. Three buses were inspected today as well as four staff and five children's files. Fire inspection: 4/13/16 Health inspection: 3/28/16 Arrival time: 6:30 am Departure time: 8:40 am

Violations:
Standard #: 22VAC40-185-550-D
Description: Based on a review of the escape drill log, it was determined that there had been no monthly practiced escape drill for October 2016. Evidence: There was no drill documented for October 2016.

Plan of Correction: We will remember to do monthly drills and to keep documentation.

Standard #: 22VAC40-185-550-H
Description: Based on interviews and an inspection of the three buses, it was determined that the center has not prepared a document containing local emergency contact information such as the closest hospitals, potential shelters and police department that pertains to each site frequently visited on field trips and pick-up/drop off of children from schools etc. Evidence: The center does not have a prepared document on any of the 3 buses of local emergency contact information for frequently visited sites.

Plan of Correction: We will prepare the document and keep in each bus.

Standard #: 22VAC40-185-580-B
Description: Based on an inspection of the three buses, it was determined that on one bus the seat was detached from the base and the seat belt was wrapped around the base and not accessible for use. Evidence: On bus # 4, the third seat back on the left was detached from the base and the seat belt was not accessible for usage.

Plan of Correction: That seat will be placed off limits immediately, all staff will be informed that the seat is not to be used and until it is repaired.
