22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted on October 31, 2016, between the hours of approximately 9:50 a.m. and 1:13 p.m. Fifty-five (55) children were enrolled and forty-six (46) children were in the care of six staff members during the on site inspection. One additional staff member was present, to assist as needed. Three children's records and two staff records were reviewed. One child's medications and authorization forms were reviewed/observed. Safety equipment (first aid kit, flashlight and radio), posted information (license, inspection documents, daily schedule, menu, emergency telephone numbers and evacuation diagram/plan/) and available documents (insurance, annual review of emergency plan and emergency evacuation drill/shelter-in-place drill log) were observed/reviewed. Snack was provided by the program (bread and carrots with dip) and lunches were provided by the children's parents. The facility was observed to be clean and supplied with a variety of age/stage program specific learning and play materials. Program observation was completed. Activities observed on this date included: children's work cycle (including, but not limited to dishwashing, puzzles, stringing beads, crayons, coloring by number, books, and math and beginning reading activities); fire drill; outdoor play; handwashing, diapering and restroom procedures; lunch; naptime; and pick-up/departure of children. Staff/child interactions, behavior guidance and daily routines/transitions were observed. Staff/child interactions were observed to be positive, supportive and encouraging of children's emerging independence. Substantial compliance continues to be maintained with the licensing standards; exceptions to compliance were noted on the accompanying violation notice. Standard #: 22VAC40-185-70-A Description: Based on review of two staff records and discussion with staff #1 on October 31, 2016, the program failed to maintain written information to demonstrate that staff #3 possessed the education required by the job position of "program leader." Evidence: 1. Review of staff #3's record, on this date, revealed that information documenting the staff member's education, as required to meet the qualifications of program leader, was not on file. 2. Staff #1 verified that the required information was not included in the staff record. Plan of Correction: The staff member was immediately notified and the information requested. Documentation verifying the staff member's education will be obtained and added to the record.
Standard #: 22VAC40-185-500-B Description: Based on observation and discussion with staff on October 31, 2016, a changing table or countertop designated for diaper changing was not used/available for children under the age of three years. The licensing inspector (L.I.) observed a child standing up during the diapering process. On this date, a child was not cleaned during the diapering process. The child was observed removing his diaper, disposing of the diaper, sitting on the toilet chair and then immediately receiving a clean diaper (put on the child by staff #2). Plan of Correction: An allowable variance will be requested for licensing standard 22 VAC 40-185-500.B.3.c. (diaper changing table or countertop designated for changing children younger than three years). Staff members involved with diapering children will be reminded that all children must be cleaned when their clothing or diaper becomes soiled or wet.
Standard #: 22VAC40-185-500-C Description: Based on observation of diapering procedures and discussion with staff #1 and staff #2 on October 31, 2016, the program failed to ensure the toilet chair in classroom AA was cleaned after use. Evidence: 1. Observation of diapering procedures, on this date, determined that the toilet chair in classroom AA was sanitized but not cleaned after use by one child. 2. Staff #2 confirmed that the toilet chair was sanitized, after use, with a bleach/water solution but not cleaned with water and soap or detergent or the use of an abrasive cleaner. Plan of Correction: Staff members who diaper children will be reminded to clean, as well as sanitize, the toilet chair after each use.
