An unannounced inspection was initiated on October 24, 2016 and completed on October 27, 2016. The inspections were conducted to investigate a complaint received by the licensing office regarding supervision, behavior guidance, and medication. Observations and interviews were conducted, and documentation was reviewed. The preponderance of evidence did not support the allegations of the complaint. The complaint was determined to be not valid. Two not reported but found violations were cited.

Description: Based on observation, the center did not ensure that children under 10 years of age were always within actual sight and sound supervision of staff. Evidence: On October 27, 2016 a preschool child was sleeping on a cot that was not within actual sight of supervising staff. The child's cot had been placed beside a wall that blocked the view of the child from staff.

Plan of Correction: This was corrected during the inspection.

Description: Based on observation, the center did not ensure that at least 12 inches of space was provided between occupied cribs. Evidence: On October 24, 2016, 2 infants were sleeping in 2 cribs that were placed against each other.
