An unannounced monitoring inspection was conducted on November 10, 2016. Eleven staff were present with fifty-six children. Children were observed participating in center activities, playing outside and doing art. Licensing Inspector reviewed standards related to supervision, staff records, fire and health inspection reports, nutrition, emergency evacuation drill log and required postings.
Standard #: 22VAC40-185-240-D-5 Description: Based on observation and review, facility did not ensure there is at least one staff member on duty that has completed daily health observation training within the past three years. Evidence: On November 10, 2016, there was not a staff member trained in daily health observation on site upon initiation of inspection at 10:20 am. A staff member arrived during inspection that has completed the training. Plan of Correction: During end of the year staff training, a daily health observation training will be done for all staff.
Standard #: 22VAC40-185-270-A Description: Based on observation, facility did not ensure areas and equipment inside and outside the facility are maintained in a clean, safe and operable manner. Evidence: On November 10, 2016, the sink in the Ladders (3's - 5's) bathroom was observed to be separating from the wall along the top seam. Plan of Correction: Sink will be repaired.
Standard #: 22VAC40-185-330-B Description: Based on observation, facility did not ensure climbing equipment or equipment with moving parts on the outside play area have sufficient fall zones under and around the equipment. Evidence: On November 10, 2016, the plastic slide had 2' of fall zone on the side of piece of equipment. Plan of Correction: Slide will be re-arranged to allow for 6'.
