Technical Assistance:
Inspector spoke with the director about anchoring of furniture in proposed infant space during this inspection.

Comments:
An unannounced inspection was conducted from 12:05pm to 2:40pm. At the time of entrance, there were 50 children present with 8 staff present and 1 administrator. The inspector observed children participating in lunch, transitioning between activities, nap/ quiet time, toileting and hand washing procedures. A sample size of 5 children's records and 2 staff records were reviewed, in addition to medication and injury reports. Areas of noncompliance are identified in the Violation Notice and were discussed with the Director at the conclusion of this inspection. Inspector also viewed, measured, and inspected the building in response to a license modification request from the center to add infant care to the license.

Violations:
Standard #: 22VAC40-185-40-E
Description: Based on observation and interview it was determined that the center did not operate within the terms of the current license issues by the department. Evidence: The center had 6 infants in care during the inspection. The current license only allows the center to care for children ages 2-12 years old. The director acknowledged that they were aware that the current license does not allow for the care of infants.

Plan of Correction: Provider response: License process iniated for this separate building on 4/12/16. Took 7 month to get through paperwork from multiple entities. Now waiting for Modification to be license under Belle Croft Preschool.

Standard #: 22VAC40-185-270-A
Description: Based on observation it was determined that the center did not ensure that all areas and equipment of the center, inside and outside, were maintained in a safe condition. Evidence: In the second building where the infants were being cared for there was a wooden stool that presented a tipping hazard to the children in care.

Plan of Correction: Provider response: Wooden stool was removed from room to storage area.

Standard #: 22VAC40-185-280-B
Description: Based on observation it was determined that the center did not ensure that all chemicals were kept in a locked place using a safe locking method that prevents access by children. Evidence: There was a can of aerosol air freshener on a shelf accessible to children in the upstairs bathroom. The air freshener was labeled "keep out of the reach of children".

Plan of Correction: Provider response: Air freshner removed from area that day- Staff retrained.

Standard #: 22VAC40-185-510-E
Description: Based on medication review it was determined that the center did not ensure that all medications with labeled with the child's name, the name of the medication, the dosage amount, and the time or times to be given. Evidence: The medications for child #6 were not labeled with child's name, dosage amount, and times to be given.

Plan of Correction: Provider response: Medication has been labeled properly and stored in locked are by Medication person. 11/1/16 Parent took and got pharmacy labels-returned 11/2/16.
