Comments:
56 Children were in care during the inspection. The age range of the children was five months to four years. Children were observed during rest time and free play. Ten staff records and ten children's records were reviewed. A sample of medication was reviewed and nine staff are current in MAT and four staff current in diabetic training. Inspection time 11:45am-2:05pm

Standard #: 22VAC40-185-330-B
Description: Based on observation not all fall zones had 6 inches of resilient surface in the fall zones. Evidence: under the climbing equipment and ladders on the older playground the mulch has become hard packed in the fall zones.

Plan of Correction: Director put in a work order on 10/2/16. Director called the facility head technician and mulch will be delivered.
