Comments:
An unannounced monitoring inspection was conducted from 10:33 a.m. - 3:15 p.m. There were twelve children in care with two teachers working directly with them. There were other school personnel available to assist as needed. Three children's records were reviewed. The record for one new teacher was reviewed. At the time of arrival the children were playing outside, collecting leaves, laughing, playing pretend, talking and engaged in discussions and negotiations, climbing, skipping and creating games. The children are participating in a naturalist class where they take walks looking under stumps and rocks to see what is living there and discuss aspects of science, nature and art. Standard #: 22VAC40-185-160-C
Description: Based on a review of staff records the facility failed to have documentation of a tuberculosis screening or test at least two years from the date of the most recent screening for one staff person.

Evidence:

1. On 11/17/2016 the licensing inspector requested documentation of a tuberculosis screening or test for staff #4 and was advised staff #4 had not provided documentation.

2. On 11/17/2016 staff #4 advised she had not gotten the tuberculosis screening or test.

Plan of Correction: Staff #4 advised she would get the required tuberculosis screening and request the office send a copy to the licensing inspector for the record.
