An unannounced renewal study was conducted from 3:30 p.m.-5:00 p.m. with the program director. 31 children, ranging in age from 5 to 12 years old, were under the care and supervision of four staff. Children were observed reading, completing homework, playing outside on the playground, playing basketball, building a dollhouse, and eating afternoon snack. Afternoon snack today was animal crackers and apples. Required information was posted. Both the fire and health inspections are current. The complete first aid kit, battery operated radio, and battery operated flashlights were present and accessible. The last emergency escape drill was 11/3/16. Two staff are currently certified in daily health observation; all four staff in first aid and CPR; and three staff in medication administration training (MAT). Three association officers' records were reviewed. Five children's records and two medications were reviewed and complete. Standard #: 22VAC40-191-40-D-1-C Description: Based on review of three association officers' records, the school did not update sworn disclosure statements for association officers every three years as required. Evidence: Association Officer A's and Association Officer B's sworn disclosure statements were dated 11/2/12. Plan of Correction: The program director stated that both officers will sign sworn disclosure statements tomorrow.
