Standard #: 22VAC40-185-60-A
Description: Based on review of children's records the center did not have all of the required information in the children's flies. Evidence: 1. During the inspection the inspector reviewed four staff files and child # 2 had only one of the two required emergency contacts.

Plan of Correction: The family service worker will contact the parent today and obtain an additional emergency contact.

Standard #: 22VAC40-185-510-A
Description: Based on a review of medications the center did not obtain the parent written authorization on all medications. Evidence: 1. During the inspection the inspector reviewed three medications. Child # 3 had two different medications at the center and the parent only signed an authorization form for one medicine.

Plan of Correction: The health director will send the form home today with the child to obtain a parent signature on the medication authorization form.
