Comments:
An unannounced monitoring inspection was conducted on Monday, November 14, 2016, from approximately 2:00pm until 5:00pm. Some, but not all, licensing standards were inspected. The following classroom ratios were observed: Infant I = six infants with three staff Infant II = eight infants with two staff Toddler I = three children with one staff Toddler II = ten children with two staff Preschool II = seven children with one staff Pre-K = twelve children with two staff Children were awaking from nap when the licensing inspector arrived. Later, the children listened to stories, ate afternoon snack and used various manipulatives. Parent/Teacher conferences were scheduled for November 28 through December 2, 2016. Four children's records, two medications and three staff records were inspected. All required emergency items were present. A practice evacuation drill was conducted 10/22/16.

Standard #: 22VAC40-185-60-A
Description: Based on review of children's records, the center did not ensure each record contained all required information.
Evidence:
Out of four children's records inspected, Child #3's record did not contain the required emergency contact name, address and phone number.

Plan of Correction: Per Center: "Will have parent put emergency contact."

Standard #: 22VAC40-185-270-A
Description: Based on inspection of the facility, the center did not ensure all areas and equipment of the center were maintained in a clean, safe and operable condition. Evidence: The following items need repair: 1. The molding behind the bathroom toilet in the Preschool II classroom is missing. 2. Several ceiling tiles in the Preschool II classroom have water damage. 3. A restroom is missing a toilet paper holder.

Plan of Correction: Per Center: "Will contact the hospital to replace the tiles. Will replace the toilet paper holder and will contact our facilities department to replace the molding."

Standard #: 22VAC40-185-500-B
Description: Based on inspection of the facility, the center did not ensure soiled diapers were disposed in an appropriate container. Evidence: In the Preschool 1 classroom, soiled diapers were being disposed in an open trash can. Disposable diapers need to be disposed in a leakproof covered receptacle that is either foot-operated or used in such a manner that neither the staff member's hand not the soiled diaper touches an exterior surface of the storage system during disposal.

Plan of Correction: Per Center: "Will purchase a pail."
