An unannounced monitoring inspection was conducted on November 16, 2016 from approximately 10:00 AM to 12:00 PM. Two staff were present with sixteen children. Children were observed playing organized games in the gymnasium and participating in various center play activities in the classroom. Licensing Inspector reviewed standards related to supervision, activities, equipment, supplies, nutrition, emergency evacuation and shelter-in-place drill documentation, fire and health inspection reports, injury reports, liability insurance coverage documentation, children's records, staff training, emergency supplies and required postings. Violations were reviewed with and verified by staff member in charge on this date during the exit interview conducted on this date. Please complete the columns for ?description of action to be taken? and ?date to be corrected? for each violation cited on the violation notice, and then return a signed copy to the licensing office within 10 calendar days of receipt. Standard #: 22VAC40-185-550-L Description: Based on review, facility did not ensure 2 of 5 parents were notified the same day the child sustained an injury. Evidence: On November 16, 2016 the following was found: 1. An injury report stated that a parent was notified on August 26, 2016 when a child sustained an injury on August 25, 2016. 2. An injury report stated that a parent was notified on November 11, 2016 when a child sustained an injury on November 10, 2016. Plan of Correction: The Center Supervisor will review the logs to ensure the date of incident is recorded accurately.
Standard #: 22VAC40-185-550-M Description: Based on review, facility did not ensure 2 of 5 injury reports have documentation of required information. Evidence: On November 16, 2016, the following was found: 1. An injury report completed on September 16, 2016 did not document the date parent was notified. 2. An injury report completed on November 14, 2016 did not document the time of the injury and the date parent was notified. Plan of Correction: The Center Supervisor will ensure all accident/incident logs are complete.
