Comments:
An unannounced monitoring inspection was completed today between 2pm and 6pm. There were 54 children in care with 12 staff. Children and staff records were reviewed. Children were observed in teacher and self-directed activities including nap, snack and outside play. A walking tour of the center inside and out was conducted. Required inspections were reviewed. Additionally, documentation for MAT, CPR, First Aid, Daily Health observation training and annual training was reviewed. Violations related to emergency evacuations, background checks, tuberculosis screening, children's records and playground were observed and appear on the violation notice.

Standard #: 22VAC40-185-130-B
Description: Based on record review and interview the center failed to obtain documentation of additional immunizations once every six months for children under the age of two years. Evidence: 1 - The the most recent immunization information in the record for child 1 is dated 3/7/17. The child's documented date of birth is 11/2/15. a. The the most recent immunization information in the record for child 2 is dated 6/5/15. The child's documented date of birth is 5/30/14. 2 - Staff 1 confirmed that the there was not immunization update information in the children's records.

Plan of Correction: Going forward center management will create and implement a system for keeping of track of when children's immunization update information is due and obtain that information from parents before it is past due.

Standard #: 22VAC40-185-160-C
Description: Based on record review and interview the center failed to ensure that tuberculosis screens are updated as required. Evidence: 1 - The record for staff 1 contains a tuberculosis screening dated 2/10/14. a. The record for the center owner contains a tuberculosis screening dated 10/14/11. 2 - Staff 1 confirmed that the tuberculosis screenings are past due for update.

Plan of Correction: Going forward center management will create and implement a system that will assist in knowing when a screening will expire so that the updated screening may be obtained before expiration.

Standard #: 22VAC40-185-330-B
Description: Based on observation the center failed to ensure that where playground equipment is provided, resilient surfacing will comply with minimum safety standards. Evidence: 1 - There is less than six feet of space between a the climb and slide and the rocking car on the playground. 2 - Staff 1 confirmed that there is less than six feet between the two pieces of playground equipment.

Plan of Correction: Since the slide is moveable center management will ensure that staff are trained to understand that due to the amount of resilient surfacing the slide may not be placed on the playground.

Description: Based on document review and interview the center failed to record a monthly evacuation drill. Evidence: 1 - The documentation provided did not include a practice drill for the month of September. 2 - Staff 1 confirmed that there was was not a drill documented for the month of September.

Plan of Correction: Center management will ensure that practice drills are documented each month.

Standard #: 22VAC40-191-40-D-1-C
Description: Based on record review and interview the center failed to ensure that background checks are updated as required. Evidence: 1 - The record for staff 1 contains a criminal history report dated 10/9/13. There is a Sworn Statement and Affirmation dated 7/10/12. The documented date of hire for staff 1 is 5/2/11. 2. The record for the center owner contains: a criminal history report dated 8/17/12, a search of the Central Registry dated 11/20/12 and a Sworn Statement or Affirmation dated 7/17/12. 3. The record for staff 3 contains a Sworn Statement or Affirmation dated 7/9/12. The documented date of hire for staff 1 is 9/29/11. 4 - Staff 1 confirmed that the background checks were past due for update.

Plan of Correction: Going forward center management will create an implement a method for knowing when background checks will expire and obtain updated information before the expiration date.
