Comments:
The licensing inspector conducted an unannounced complaint inspection in response to a complaint that was received by the licensing office on September 21, 2016 and initiated the same day. On site interviews and telephone interviews were conducted on 10/18/16 and 10/27/16. The investigation is in response to allegations of lack of following required ratios. The information gathered during the investigation supports the allegations, therefore, the complaint is valid. Violations were found in the areas and appear as violations on the violation notice: ratios. A violation in the area of administration was found in course of the investigation.

Description: Based on interviews and review of documentation the center has failed to demonstrate operational responsibility regarding ensuring the center's activities services and facilities aer maintained in compliance. Evidence: 1 - During interviews staff explained their method of counting ratio and their understanding of the face-to-name sheet. However, the information shared by management was different and affected the whether or not an additional staff person was needed in a classroom. 2 - During interview 3 out of 9 staff interviewed stated that the room they work in are routinely out of ratio. Staff stated that they had communicated concerns to management. 3 - During in interview center management confirmed that a review and training for all staff regarding ratio counts, communication with management and use of center ratio documentation needed to be reviewed.

Plan of Correction: Center management has implemented the following improvements: - update name to face sheet to reflect the number teachers present - adjusted staff hours so that additional staff are present during the early morning hours - will hang ratio signs in each classroom as a reminder to staff and parents - retrained ratios with staff during staff meetings - will engage staff in ongoing training - there will be cameras installed throughout the building - management staff is more available to assist in classrooms.

Description: Based on interview and review of center documentation the center failed to ensure that when children are in regularly ongoing mixed age groups the staff to child ratio applicable to the youngest child in group applies to the entire group. Evidence. 1 During interviews staff report that in the morning rooms are frequently out of ratio a. staff 1 reports that on the morning of 10/18/16 at 8am there were 10 children in care 11months through 2.5 years old. However, the third staff did not arrive until 9:35. b. staff 2 reports that approximately five times since she began working for the center in April, 2016, the infant room has been out of ratio. c. staff 4 reports that 80% of the time when she arrives at 7:00am there are 7 children present in the woddler room with staff 3.
