22VAC40-191 Background Checks (22VAC40-191)

Technical Assistance:
Discussed the following as consultation with Center Director: Hand washing procedures, location of step on diaper pail, completion of daily disposition forms for children, air freshener plug ins, water temperature, securing laundry room door, cleanliness and maintenance of main sink in infant room, heating vents. You are reminded not to post the supplemental page of the inspections. Transition from one program activity to the next.

An unannounced monitoring inspection was conducted from 8:30 am-1:00pm. At the time of entrance, 136 children were in care with 20 staff members present. Children were observed during snack time, engaged in group activities, diapering, hand washing and outdoor play time. A selection of staff and children records, medications and other documents were reviewed. The violations were reviewed with the center director. Standard #: 22VAC40-185-160-B
Description: Based on staff record review, licensee did not obtain acceptable form of documentation of TB screening. Evidence: Staff #1 hired 09-14-2015 did not have an acceptable form of TB screening. Staff's TB screening on file showed a positive PPD reading recommending a chest x-ray. There was no documentation of the negative chest x-ray on file.

Plan of Correction: Staff has provided documentation of a negative TB test through a chest x-ray.

Standard #: 22VAC40-185-270-A
Description: Based on observation the center's playground fencing was not maintained in a safe condition. Evidence: Fencing on the toddler playground off of room 4 was found with cracked and peeling paint on several boards. Fence also had protruding nails and one board split with sharp edges.

Plan of Correction: The fence board that was split has been replaced as well and the protruding nails have been fixed (11/14/16). We will be stripping the paint and re-painting the fence. (date to be corrected: by 12/5/16).

Standard #: 22VAC40-185-520-C
Description: Based on observation in both infant rooms diaper ointment found not labeled with the child's name. Evidence: Child #1, #2 and #3's had ointments not labeled in their bins.

Plan of Correction: All diaper ointments were corrected with the appropriate label, to include child's full name, during the inspection.

Standard #: 22VAC40-185-560-G
Description: Based on observation in infant room #2, food brought from home was not labeled in a way that identifies the owner. Evidence: Child #5's bin had packaged food that was not dated and labeled with the child's name.

Plan of Correction: All food brought from home were corrected with the appropriate label, to include child's full name during the inspection.

Description: Based on observation in infant room #2 bottles not dated. Evidence: Child #4's bottles were not dated.

Plan of Correction: All bottles were corrected with the appropriate label, to include child's full name, during the inspection.
