1. Discussed documenting staff position changes, children's files updates to better access information (allergies and start date), and staff qualifications.
An unannounced renewal inspection was conducted today from 9:30am-12:20pm. There were 32 children directly supervised by 6 staff. The physical plant, 2 staff records, 4 children?s records, evacuation drills, injury reports, and policies were inspected. Children were observed participating in group play, playing outside, and circle time. Hand-washing procedures were also observed. There were no medications on site. There was an abundant supply of materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-60-A Description: Based on record review 3 of 4 children's records were incomplete. Evidence: 1. Children #1, #2, and #3 were missing guardians work phone numbers. 2. Child #1 was missing both emergency contacts. The parents were listed. 3. Children #1 (lasted reviewed 9/26/15), #3 (last reviewed 9/22/15, and #4 (last reviewed 9/22/15) did not have the annual review of emergency information documented. Plan of Correction: I will get the missing information.
Standard #: 22VAC40-185-70-A Description: Based on review 1 of 2 staff files were incomplete. Evidence. 1.Staff #1 did not have 2 references with all required information. The references available did not have the date of contact or the signature of the person making the contact. Plan of Correction: We will review out form.
Standard #: 22VAC40-185-240-C Description: Based on record review 2 of 2 staff, hired after June 2008, did not have 16 hours of annual training. Evidence: 1. Staff Staff # 2 (hired 6/25/2012) only had 14 hours of annual training for the 2015-2016 operational year. Plan of Correction: I will encourage staff to get training.
