An unannounced monitoring inspection was conducted from 10:10 a.m. - 1:15 p.m. This was the first time this licensing inspector has worked with this facility. There were fifty children in care with eight teachers working directly with them. There were other school personnel available to assist if needed. Four children's records were reviewed. The record for two teachers were reviewed. The children were observed during circle time, writing the letter F in the air, singing from memory while keeping the tempo with drum sticks, writing in journals about what they are grateful for, during hand washing and toileting, drawing and coloring, building with blocks, engaged in discussions and during negotiations with one another, listening to a story, during the Thanksgiving feast wearing their Pilgrim and Native American attire and during nap. Standard #: 22VAC40-185-160-A Description: Based on record review the facility failed to have prior to or no later than 21 days after employment documentation of a tuberculosis screening or test for one staff member. Evidence: 1. On November 22, 2016 the licensing inspector reviewed the record for staff #2 and there was no documentation of a tuberculosis screening or test in the record. 2. On November 22, 2016 staff #1 reviewed the record for staff #2 and confirmed there was no documentation of a tuberculosis screening or test in the record. Plan of Correction: I contacted the staff member immediately and we can have documentation of the tuberculosis screening/test in the record by next week.
